If students are unable to attend a final exam due to official business, bereavement, childbirth, pregnancy, care for children below the age of three or other unique circumstances they must obtain prior permission from their course instructors and fill out the leave request form which can be downloaded from the webpage of the Student Assistance Section of the Office of Student Affairs. After completion of the approval process (course instructor→submitted to academic advisor and department dean within 7 days→submitted to the Dean of Student Affairs after more than 8 days) the third copy of the form should be submitted to the Curriculum Section to complete the leave application procedures in order to be eligible for a make-up exam.
